First, make sure everything looks like it should (sounds simple, but even after years of experience, you'd be amazed at the little things that trip you up cause you overlook them). Check ferrules, housing and the lever.
Next, get over the caliper and watch the pistons as you pull the lever...
If you're riding trails, you can use a thicker chain lube, since the things are usually wetter. Thicker, deeper tire tread is also helpful. If you're rockin a road bike, a more viscous lube in the shifters and housing can be helpful when things really get chilly.
^^exactly. If the 203 rotor is brand new, then you'll have to brake it in. After the sandpaper/rubbing alcohol, find a big hill and make a few trips down it with the brakes on.

Integrated is a style, Aheadset is a brand, and unfortunately, your frame makes your decision for you. Measure the inside diameter of the headtube (top and bottom) and go from there.

Are you sure it's threaded? I have seen the '99 hardrock and it came with a threadless headset. But there are 1-1/8" threaded headsets out there so that might be a possibility also. Why do you want to switch from threaded to threadless? Unless you change steerer tubes (or forks) I think you have...

definitely replace it, once your ht gets that tweaked that you can pull the cup in and out, its pretty much done. Its always a shame when you try to build up an old frame and its got some damage... been there, done that, got the worthless piece of sh1t sitting in my garage to prove it.

how laterally stiff are Lefty "forks"? I'm coming from a gold label 80mm with a thru axel so pretty much anything feels like a flex chicken in comparison, but how well do Lefty "forks" compare with typical XC forks?
