sadlly Tandem shells are far to small in diameter (only about 1 1/8" of adjustment) and they don't provide sufficient range of adjustment

your problem is exactlly what i do for a living, exept I do it for motorcycles...
if you can please post a few pictures of you on the bike, a side view when you are sitting, another standing and if is possible a frontal view to see the size of your shoulders and also a view...
also a beautyful side-effect of a "Close" hydrolic sistem (No Air inside or a expansion chamber only DOT fluid) is that you can run them Upside down if you need to, like I did for my tandem brakes...
plus the brake mount piston (radial, a.k.a. perpenticular to the handlebar) or the brake hose...
