Not replacing the worn bearings?
Re-fitting too few bearings?
Not re-adjusting the cones (adjust inside nut, then tighten against locknut) properly?

Give us a clue here, eh?

Bearings looked fine so I repacked them and reassembled. Tons of play. Tried it again doing what seemed like exactly the same thing and they're totally fine now. I have no idea but at least it's back together and working. Definitely need a thinner 10mm wrench the next time because it's awfully hard to tighten the locknut against the cone with regular wrenches.
