Stall from hell or massive system failure on board, I'm leaning toward pilot error for now as the 747's pretty much define "reliable".

My boss is there now...fixed and rotary winged aircraft have been falling out of the sky on a distressingly regular basis lately. Thats what happens after 10+ years of regular high stress activity...
