The "best" b-rated horror I've ever seen was called Truth of Dare.

I saw it in the late 80s-early 90s, but it might have been a few years old by then.

It involved a guy going crazy for some reason, and truth ing and/or daring people to death. The mental patient scene with the grenade in the mouth was memorable.
