so today I replaced my cassette,chain and middle chain ring on my xt cranks, the rear shifts like butter, the problem is when I am in the middle chain ring and mash hard the pedals the chain flies off the ring and on to the small ring or comes all the way off , the big and small rings work great, the front derailer seems to be working right, the chain is well lubed, the rings are alinged properly. The replaced ring is the same as the original, the chain is a sram, any ideas?

is your chain rubbing your front derailleur at all while you're in that middle chain ring?

If you replaced it with a shimano middle ring, that could be your problem. Seems there is a bad batch of middle rings out there.
