Seriously though:

If it isn't shifting up, turn the barrel adjuster counterclockwise.
If it won't shift down, turn it clockwise.
Do smaller and smaller turns each time, don't get frustrated, it's hard to do perfect.
Now someone else explain limit screws!

ioscope is confusing things a little. what he calls shifting up, is actually down-shifting. and what he calls shifting down is actually up-shifting. higher gear is harder to pedal, lower gear is easier to pedal. limit screws on the rr derailleur are marked with an L or an H. they control how far the derailleur can move past their high and low limits. screwing them clockwise restricts/limits their movement past the last cog on either end of the cassette. screwing them counterclockwise lets the derailleur move farther out on each outer edge of its range.

OK... here's my very unscientific aproach to der adjustment.
  • start with the chain off and the der cable "unhooked" (i.e. loosen the bolt on the der to free the cable)
  • shift the bike to its hardest gear
  • adjust barrel adjusters so they are in the middle of their adjustment range
  • at the der, pull all the slack out of the cable
  • give it a pull, hold it there, and tighten the bolt down to secure the cable
  • shift through the gears a couple of times
  • with the bike in its hardest gear, check to see that the first/top der pulley lines up with the smallest cog. If it doesn't, adjust the H limit screw - screw it in to allow less movement (if the pulley is outside the cog), screw it out to allow more (if the pulley is inside of the small cog)
  • shift the bike to it's easiest gear
  • check to see if the pulley is lined up with the biggest cog. If not, adjust the L limit screw the same way you did in the previous step
  • install chain
  • shift into hardest gear
  • check chainline as the chain comes off the smallest cog and goes around the pulley. If it's not straight and smooth when pedaling, use your barrel adjuster to correct it. Just play around until you get it right (I said this wasn't very scientific).
  • shift into one of the middle-ish cogs
  • check the chain line again, use the barrel adjuster if necessary to tweak chain line
  • shift into the easiest gear, check chain line.
  • once everything looks good, take it for a spin up and down the driveway... make sure the shifting is smooth with weight on the bike. If not, use the barrel adjuster to smooth it out.

A couple of notes... the limiting screws are really only good for rough adjustment - i.e. getting thing close if you're way out of wack or if you've installed a new der or cable. The barrel adjuster is what will get things dialed in. BE PATIENT... I've been at this for over a year, and I still can't get it on the first try all the time. Some times you just have to play around to get it really dialed in.

All good info but seems to me you should start by looking at the hanger cause if your hanger is bent inwards you can fvck with the adjustment till your blue in the face and it will not work out. a bent hanger will cause ghost shifting up and down all by itself.
make sure your der. hanger is straight if not insert an allen wrench in the der. mounting bolt and just tweek it back then go through the adjustments as listed above
