To be fair that did happen in the late 70's when mental health issues were treated with a slap and the direction to 'get over it'

Don't worry, Dave's just dirty cos we won the cricket today.
There was a case in Japan where one of the pilots went barmy and crashed the plane into Tokyo Bay in the 70's. Edit, in 1982
Japan Airlines Flight 350 - Wikipedia, the free encyclopedia@@AMEPARAM@@/wiki/File:Translation_arrow.svg" class="image"><img alt="Translation arrow.svg" src="http://upload.wikimedia.org/wikipedia/commons/thumb/2/2a/Translation_arrow.svg/44px-Translation_arrow.svg.png"@@AMEPARAM@@commons/thumb/2/2a/Translation_arrow.svg/44px-Translation_arrow.svg.png
The cause of the crash was traced to Captain Seiji Katagiri's deliberate engaging of the number 2 & 3 engine's thrust-reversers in flight, in an attempted suicide. The First Officer and Flight Engineer worked to restrain him and regain control. Despite their best efforts, the DC-8's descent could not be completely checked, and it touched down in shallow water 300 meters (1,000 ft) short of the runway.
