Thats a shattered pyrex sight glass that when not shattered, allows you to see the ammonia liquid level in the high pressure receiver.

That broken glass should have been the path the entire liquid charge dumped out of the system.
Why didn't the system dump? Empty to begin with or did someone's vigilance catch it before the big uh-oh moment?

Neither, system was still running. I wandered around in the room looking for the source of the stink for 10 minutes. I don't know why it didnt dump. The tank above it is showing a healthy liquid level.

Either the liquid isolation valve has an emergency flow restsrictor (??) Or the tank is old enough that some old rust/gunk plugged the opening.
