I need to start getting my camelback in order so I'm not one of those unprepared people when something bad happens.

so far in my pack, just on normal rides, I keep:

spare tube
Patch kit
tire levers (those crappy plastic things we all hate)
pump
5, 6 and 8 mm allen wrench
chain breaker (POS that I need to replace)
red bull (for that extra burst of energy when I REALLY need it)
some type of food such as power bars or something

I need to get:
a first aid type kit (bandages, and ? not sure what should be in it)
duct tape
zip ties
bug spray

what am I missing/forgeting?

I carry all that stuff, plus an extra chain - I've lost entire chains on rides before and it sucks! I also always carry a good pocket knife, just in case. Perhaps sunscreen? Chapstick, especially on cooler and/or windier days?

On longer rides, I carry a second tube and a spare der hanger.

ditto on the sharp/good pocket knife. something like a leatherman ought to be perfect and cover the other tools that your multi tool doesnt have.

bare bones for me tape, gauze, a wrap (use it for sling too). Then the space blanket is good not only for having to spend the night in the sticks, if things go bad, but for wrapping someone up who's going into shock.

WHERE do you put all that stuff? and when do you take that much stuff with you?
you should have the squirt gun in a holster...... :p
